Carers Support Plan
The Borders Carers Centre have been responsible for carrying out the Carers Support Plan (previously known as the Carers Assessment) since 2012. The name was changed from 'Assessment' to
'Support Plan' in 2015, and we immediately saw a 25% increase in the uptake of carers agreeing to a Support Plan.

What happens next?
The Carer will receive a letter from the Carer Liaison Worker allocated to them (based on location). They will be advised that the worker will be in contact within 6 weeks.
If the carer would like a Carers Support Plan, the Carer Liaison Worker will arrange to speak to the carer in private as soon as possible to complete the Support Plan. The carer should allocate approximately 2 hours.
The Support Plan is designed to identify support for the carer in the caring role as well as recognising personal needs resulting in a person-centred carers' action plan.
If you wish to see a copy of the Support Plan, or be kept updated with its' progress, we will need permission from the carer.
Every carer has the right to a support plan, and must be offered this. Even if the carer does not meet the eligibility criteria, they are still entitled to support and information, which is offered by the Borders Carers Centre.
