​Please contact us if you would like to arrange a visit with our Hospital Liaison Worker, or ask the staff on the ward at Borders General Hospital or in any Borders Community Hospital.

​When a  loved one is admitted to hospital, it can be a daunting time.  

Our Hospital Liaison Worker, who works in partnership with Borders General Hospital and Community Hospitals, is there to ensure this stay and the transition to discharge is as smooth as possible.

She visits the wards and holds carer one-to-ones regularly in order to fully understand your personal circumstances and make you aware of your rights. 
​
She will  then work with you intensively whether your loved one is going home from hospital or not.
