Support Plan

The Carers (Scotland) Act 2016  entitles all carers  to an assessment (now referred to as a Carers Support Plan) in their own right.  This means that all carers can now ask for their own needs to be assessed in relation to the perso​n they care for.  A carer has the right to have this, even if the person they are caring for does not wish support for themselves.

What is the Support Plan
What are the benefits to me?
A chance to have a chat with a Carers Liaison Worker about your own personal circumstances.
​It will help you identify your own needs and make decisions, as well as looking at the help and support you may need to enable you to continue caring, if this is what you choose to do.
Tell me more....
 It may involve one of more visits to you in your home or a suitable venue of your choice.  The Carers Liaison Worker will explain what will happen in more detail and will be pleased to answer any questions you may have. Please allow up to two hours for the support plan to be completed. ​

​How do I get a Support Plan?

You can get a Carers Support Plan by contacting the Borders Carers Centre directly.  The staff in the Carers Centre will complete a form with you to help you identify what would help you.   Referrals are also made to the Carers Centre through Health & Social Care and other agencies.

What happens next?

You will not have to pay for any services provided by the Carers Centre. 

The Carers Support Plan will identify areas where you may benefit from some support or assistance.  This may be assistance to apply for extra funds, essential aids and equipment, a holiday, respite, a referral on to other services or a benefits check.  You will also be given plenty of emotional and practical advice, as well as access to peer support groups, days out and special events to help alleviate some stress.   In short, whatever you need to support you in your caring role.

Social Work Assessments


If it is identified that you, or the person you care for,  could benefit from a Social Work Service, and with your agreement, the Carers Centre will refer you to Social Services for an Adult Core Assessment.  This is a SEPARATE assessment, and is not part of the Carers Support Plan. If you are assessed by Social Work as needing a service, you will be asked to give information about your finances.  You may qualify for a free service, or you may have to pay part, or the full cost of services.  You will be told of any costs before services are arranged to give you a chance to consider whether you want to accept the services or not.
