When a loved one is admitted to the hospital, it can be an overwhelming and stressful experience.
Our Hospital Support service is designed to provide comprehensive assistance to both carers and their loved ones during hospital admissions and the transition back home or to another care setting. Our dedicated Hospital Liaison Workers collaborate closely with Borders General Hospital and Community Hospitals to ensure that the admission and discharge process is as seamless as possible.
Whether your loved one is returning home or transitioning to another care setting, our team is here to provide intensive support tailored to your needs.
